Rule Violated: Daily records of arrival and departure times for children enrolled at the center were not maintained and available for review.Four (4) children were signed in for the day, however fourteen (14) were present in space 2.

Rule Violated: The center did not complete an EPR Plan within four months of completing the EPR in Child Care training and/or the Plan was not completed on a template provided by the Division. The EPR plan was not printed off. It was completed however no hard copy was on the file at the facility.

Rule Violated: The child care administrator and all staff did not complete the Recognizing and Responding to Suspicions of Child Maltreatment training within 90 days of employment. One (1) employee started employment August 13, 2019, she has not taken the Child Maltreatment as of today.

Rule Violated: An incident report was not completed and mailed to a Division representative within seven days after the incident when medical treatment was required. I found an incident report that was date February 19, 2019. The incident report stated that EMS was called to the facility. This incident report was not mailed to me.
