Rule Violated: Running water, soap and individual sanitary towels, or other approved hand-drying devices were not supplied at each lavatory. There was no soap or toilet paper in the bathroom used by school-age children.

Rule Violated: All equipment and furnishings were not in good repair. Several items on the playground used by the school-age children were not in good repair: 1. The playhouse is cracked on the inside; 2. Three(3)seats on the Merry Go Cycle are cracked and one (1) pedal is missing (the spoke is exposed); 3. There are spokes raised up on the railing along the shelter; 4. Several balls are flat or busted; 5. The chains, hanging below the swing seats, need to be cut off or made inaccessible to the children.
