Rule Violated: All staff, including the director, did not have an annual health questionnaire on file. Two of the thirty-two staff files reviewed didn't include a current health questionnaire

Rule Violated: New staff, who had contact with children, did not receive at least 16 hrs. orientation within first 6 weeks. Three of the thirty-two staff employed had not completed the required orientation training within the first 6 weeks of employment.

Rule Violated: All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. Two staff members didn't have a current FA course.
